The Oregon Health Authority defines health-related social needs, or HRSN, as "social and economic needs that affect your ability to maintain health and well-being."
We are partnering with IHN-CCO to connect Medicaid eligible members with these additional wrap around supports. HRSN benefits can include support for food, housing stability, utilities, transportation, and other essential services, recognizing that health is influenced by more than medical care alone. These needs arise from broader Social Determinants of Health (SDOH) but are addressed at the personal level by connecting people to resources like housing assistance, healthy food programs, and support services to improve health outcomes.
Eligibility is assessed on a case-by-case basis for OHP members who meet criteria related to housing instability, food insecurity, medical or behavioral conditions, and income restrictions.
We can help connect you with housing and climate supports if you meet all eligibility criteria.
We provide evidence based health and nutrition education for OHP members who meet all eligibility criteria.
